A prescription-strength anticonvulsant from Zydus Pharmaceuticals, Topiramate 25?mg is formulated to manage epilepsy and prevent migraine attacks. It works by stabilizing electrical signals in the brain, reducing the frequency and severity of seizures and migraine episodes 

Key Indications

• Monotherapy or adjunctive therapy in epilepsy: Effective for partial-onset and generalized tonic-clonic seizures in pediatric (2?+ years) and adult patients 

• Migraines prevention: FDA?approved for migraine prophylaxis in patients 12?years and older

Mechanism of Action
Topiramate exerts its effects through multiple pathways:

• Enhances GABAergic inhibition and attenuates glutamatergic excitation (AMPA/kainate receptors)

• Blocks voltage-dependent sodium channels

• Inhibits select carbonic anhydrase isozymes
  Together, these actions reduce neuronal hyperexcitability, controlling seizures and migraine activity 

Dosage & Titration

• Starting dose: 25?mg once daily, typically in the evening

• Titration: Gradual increase (e.g., by 25–50?mg/week) guided by therapeutic response and tolerability; adult maintenance range often 100–400?mg/day, divided Migraine prevention: Gradually titrated to 100?mg/day in two divided doses tailored to response .

Safety Profile & Warnings

• Oligohidrosis, metabolic acidosis, kidney stones, and visual disturbances are noted in some patientsMood changes, cognitive effects (e.g., memory issues, slowed thinking), and suicidal ideation may occur; caution during dose changes 

• Use in pregnancy is linked with teratogenic risks (cleft lip/palate) and small-for-gestational-age infants

Administration Guidance

• Swallow tablets or capsules whole, with or without food.
